After finishing both of their loyalty missions. Shepard is informed by Joker that Jack and Miranda are in a fight. On my first playthrough I had a problem keeping both of them loyal although my then Paragon bar was nearly full. The problem here is that you don't need a full Paragon or Renegade bar for that. On my current playthrough I finally managed to video capture it. It actually depends on at what point you are in the game and how many paragon/renegade points you can have gotten so far and how many percents they are compared to the total Paragon/Renegade points you can have at this stage of the game. It is explained here so you may want to check this out.
Also very important is to MAX out your special talent first! I included my Squad screen in the video so you can see that my Shepard is only a level 14 Sentinel but has the Guardian talent maxed out already, and my paragon/renegade bars are FAR from being full. In addition to that, I also maxed out the special talents for Miranda and Jack while playing the game. So it's helpful to do both of their loyalty missions as early as possible. The longer you wait, the harder it gets. As you can see from the Team Status Screen I haven't recruited everyone yet, and only done Jacob's and Miranda's loyalty missions so far. The video in the beginning is right before entering Jack's cell, which is the last stage where you make a save, if you want to take screen caps or video caps. You should make a save before starting the Pragia mission so in case, solving the fight doesn't work for you, you can reload that save and either retrain your powers (to re-arrange your talent points) or do another mission to get more talent points. I hope this helps. If you have questions, feel free to ask.
This is from a renegade playthrough. Of course it works the other way around as well.
Check the SocialBioware forums too. Lots of information there....
